This waiver must be agreed upon for each booking at the club by clicking “yes” to the acknowledgment listed in our online booking website and app. 

​No booking or program will be accepted without the confirmation of the waiver and declaration prior to each booking.

COVID-19 MEMBER WAIVER AND DECLARATION

This waiver and declaration must be completed prior to visiting The Supreme Court each time a member reserves a court, program or any other activity at the club. A verbal declaration may also be required on every visit to the Club. This Declaration is provided in addition to the terms and conditions which you have agreed to in your membership agreement. Please refer to this form for further details including your existing release of the Club.

The Club appreciates your cooperation in following the COVID-19 protocols that have been established to protect Members, Staff, and our community. These protocols have been established based on recommendations from the Province of Ontario, health and safety association guidance and sport governing body recommendations.

The Club is reopening based on the assurances Members make to the Club and to each other in this document and on each visit to the Club. Accordingly, any false statements could result in the Club being forced to deny Member access. Further, Members agree to observe and fully abide by all rules, protocols and procedures set out by the Club. As we continue to monitor the situation regarding COVID-19 in Ontario and Toronto, we will continue to review protocols and procedures and adjust accordingly.

The virus that causes COVID-19 can be transmitted by asymptomatic people and there is a risk of transmission in spite of all of the Club’s best efforts to impose and enforce appropriate safety protocols. Each Member must assess themselves, and if they choose to visit the Club, take on the risk of contracting the virus and transmitting it to others.

All information will be stored, processed, and used in accordance with the Club’s membership agreement available for your review at thesupremecourt.ca/agreement

By completing and submitting this Declaration you agree to the following:

RELEASE – PLEASE READ CAREFULLY:I UNDERSTAND THAT CHOOSING TO ATTEND THE CLUB INVOLVES RISKS, INCLUDING WITHOUT LIMITATION, BODILY INJURY OR HARM AND CONTRACTING COVID-19. I ACKNOWLEDGE THAT I AM PARTICIPATING VOLUNTARILY. I FREELY ACCEPT AND FULLY ASSUME ANY AND ALL OF THE RISKS, WHETHER CAUSED BY THE NEGLIGENCE OF THE CLUB OR OTHERWISE. I RELEASE THE CLUB, ITS DIRECTORS, OFFICERS, EMPLOYEES AND REPRESENTATIVES AND ITS LANDLORDS (GOODLIFE FITNESS CENTERS) FROM ANY POTENTIAL CLAIMS OR LIABILITIES RELATED TO MY ATTENDANCE AT THE CLUB.

I hereby declare that: (a) Neither I nor anyone in my household have experienced any cold or flu-like symptoms in the past 14 days (including, without limitation, fever, cough, headache, sore throat, respiratory illness, or difficulty breathing); and (b) To my knowledge, I have not been in close contact with anyone with flu-like symptoms or acute respiratory illness in the past 14 days. *

If I or anyone in my household experience any cold or flu-like symptoms or receive positive test results for COVID-19 after submitting this Declaration, I will: (a) immediately inform the clubs Tennis Director, Alastair Millar (); and (b) not visit the Club until I receive clearance to do so from a qualified medical professional, or fourteen (14) days after the cold or flu-like symptoms have completely gone away, whichever is longer.

I hereby declare that I or any member of my household have not travelled to or had a lay-over in any country outside Canada in the past 14 days. If I or anyone in my household travel to any country outside Canada after submitting this Declaration, I will then not visit The Supreme Court for a minimum period of 14 days after the date of return to Canada.

By clicking “yes” to this agreement this means you have read and understand this “THE SUPREME COURT COVID-19 MEMBER WAIVER & DECLARATION”.

I hereby agree that any false statements made in this Declaration or any failure to abide by the above noted requirements may lead to disciplinary action, which may include restriction of privileges, suspension of membership or expulsion from the Club.
